I/we, the parents or guardians of the above named child, hereby give my/our approval to his/her participation in any and all of the activities of the ORYA program. I/we are aware that participation in this program could cause cardiovascular harm, injury, heart attack, or even death to my/our child. I/we assume all risks and hazards incidental to such participation, including transportation to and from the activities. I/we do hereby waive, release, absolve, indemnify and agree to hold harmless ORYA, the organizers, participants, sponsors, coaches, the towns of Durham, Lee & Madbury and the persons transporting my/our child. I/we certify that all known physical defects pertaining to the above applicant have been enumerated.

In case of accident or serious illness suffered by my/our child during his/her participation in an ORYA activity, I/we hereby request the ORYA agent (s) to contact me/us at the above telephone numbers. In the event that I/we cannot be reached, I/we authorize ORYA or its agents to secure whatever reasonable and necessary medical treatment is required.

We want every member to have a positive and enriching experience. Therefore, please be considerate of those around you. ORYA will not tolerate any behavior affecting the comfort, enjoyment, health, safety or well-being of other members, officials or opponents. We reserve the right to refuse, suspend or revoke the privilege of membership in ORYA and the right to participate in ORYA activities to anyone who, in the judgment of the ORYA, is conducting or has conducted themselves in a manner that adversely affects the experience of others. Please refer to the ORYA Policy on Conduct and Harassment for more details on this important policy.

I, as a player, coach, parent, spectator, official, pledge to conduct myself in a manner that complies with the ORYA “code of conduct” at all times. Accordingly, I pledge to:

• Lead by example by demonstrating respect to other players, coaches, parents, officials and spectators as well as encouraging good sportsmanship and fair play.

• Support the drug, alcohol, tobacco and crime free environment that is important for all youth sporting events.

• Focus on fun and participation

• Understand that the safety and welfare of all are concerned is the top priority.

Further I pledge NOT to:

• Ever use profanity at a youth event

• Criticize coaches, officials, players, or spectators

• Touch a player, coach, official, or spectator in a threatening manner.

This pledge must be signed by any player, coach, parent, or official who wants to participate in any ORYA event.

By signing this document, I agree to abide by and uphold the above stated ‘Code of Conduct’. I understand that violation of this code may subject me to disciplinary action by ORYA and possible expulsion from ORYA.

I/we give permission to ORYA to use photographs of my child for promotional purposes on the website, in newsletters and on other promotional materials. Upon request, I/we have the right to not allow photos of my/our child to be used for the above.
